Medical Release Forms

Hello all- I have some technical (but actually pretty important) information to convey to you all. I want to direct your attention to the fact that in order for your child to participate in any activities that are sponsored by Hillcrest and occur off-campus (ie anywhere other than the church grounds) they are required (by legal stuff, not by me) to have a completed and signed Medical Release Form (MRF) on file at the church office. Most of the students that are actively involved at this point have filled this form out for this year, however, there are a few that still have not. This form can be found on the church's website here.

If you are not sure whether or not the church has a completed MRF on file for your student, please call the church office and speak to Dustin or Judy. If we don't have one on file, please download a form from the church's website, complete it, sign it, and return it to Dustin or the church office during normal business hours. Thanks!
